Xu Tianzhi was born in Hangzhou in 1927 into a family of traditional Chinese medicine. Influenced by her family and her passion for medicine, she paid a visit to a teacher at the age of 2. At the age of 19, she was admitted to Southeast Medical College in Shanghai. Shanghai Southeast Medical College was the predecessor of today's Anhui Medical College.

The family background of three generations of traditional Chinese medicine, combined with Western medicine education at school, allows Xu Tianzhi to view problems from a more comprehensive perspective. At school, Xu Tianzhi got excellent grades and was the first one almost every year. After entering Zhongshan Hospital, she chose pediatrics without hesitation. As an intern, Xu lived 24 hours a day in a hospital, taking care of a patient from start to finish. She could not go home for weeks, and it was her first experience of a doctor's responsibility.

In 1952, Xu Tianzhi, a Hangzhou native, came to Nanjing from Shanghai with her husband in response to the country's call. At first, she was assigned to Nanjing Gulou Hospital to train as a general practitioner. In the following year, the department of Pediatrics was separated from Gulou Hospital and merged into the former Sacred Heart Children's Hospital on Guangzhou Road. Xu Tianzhi, who was a resident doctor at that time, was also transferred to the children's hospital.
